Our specialist supported living and outreach services, provide support for people with Mental Health needs, Autism, Learning Disabilities and Acquired Brain Injuries. We understand the importance of providing person-centred support, which enables independence and meets individual’s needs. Through our outcome focussed approach, we support people to maintain choice and control of their own lives. We meet with each person referred and their family so that we can develop a strong understanding of their life story, any support or communication needs and their personal aspirations.
We work in partnership with local health professionals such as social workers, community nurses, and specialists such as speech and language therapists, physiotherapists and occupational therapists.
Here at Trust In Care, we have highly trained and dedicated staff to provide support and care for individuals with
Autism, Learning Disabilities and Mental Health needs. We take pride in providing the very best in personalised
care as we understand how important it is to really highlight an individual’s independence, choice, health,
well-being, dignity and respect.
Our friendly management team and care co-ordinators will take the time to reach out and meet with clients and their families to gain a better knowledge of an individual’s care needs and personality, so together we can build a care and support plan which is person-centred.
Trust In Care are here to also support the family members and/or main carers of an individual, as we understand the brilliant work you are doing but you might need that well deserved break. We work closely with other professional
services within the community to ensure all care and health needs are being met. Our goal is to encourage
choice and independence and to promote health and well-being.
Our staff support each person to create a person-centred plan that tells us how they
want to live their life now and in the future. Through a range of communication tools,
we ensure that each person is closely involved in the development of their own plan.

including overnight support when needed. This can be provided within the family home
or a person’s own supported tenancy.
